Million Hearts

Million Hearts® 2022 is a national initiative co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS) to prevent 1 million heart attacks and strokes within 5 years. They have tools for practices to use for heart and stroke patients.

SafeMed

The SafeMed care transitions model uses intensive medication reconciliation and home assessments to manage the patients who tend to be heavy utilizers of inpatient and emergency services. The University of Tennessee Health Science Center developed this toolkit in collaboration with the American Medical Association.

SmokefreeTEXT App

Your patients can sign up for a free texting program to help them quit smoking through the National Cancer Institute’s Smokefree.gov program. Patients will start by accessing the sign-up page and then they can create their own texting program that works for them.

TN Pop Health Consortium

The Consortium is a collaboration of major academic institutions, healthcare providers, and other stakeholders working to improve population health and health equity across Tennessee. TN Heart Health Network is the Consortium’s signature project.

AHRQ Evidence Now

The Agency for Healthcare Research and Quality (AHRQ) EvidenceNOW program seeks to improve heart health and help reduce cardiovascular disease disparities by disseminating and implementing patient-centered outcomes research (PCOR) findings in primary care practices to improve care delivery.
